ROGER SHARRER HOCKEY DEVELOPMENT GRANT

APPLICATION

 

 

PURPOSE

Funds received from the Roger Sharrer – “Grow Hockey” Grant shall be used exclusively to grow or increase participation in recreational ice hockey.  All local hockey associations within the Mid-American District of USA Hockey are eligible to apply.  Applications are due and must be postmarked by April 11, 2008.  Grant applications should be mailed to:

Grants of up to $4,000.00 will be awarded to local hockey associations, who in the opinion of the Mid-American Hockey Association Board, offer the best plan(s) for growing participation in recreational ice hockey in their area.  Successful applicants will be notified by June 1, 2008 and will recognized and receive their award shortly thereafter.  Grant recipients will be recognized at the Mid-American District Annual Meeting in Columbus, OH in June.  Grants awarded in June 2008 will be for the 2008-09 hockey season.  Grant recipients agree to provide periodic updates as to how funds are being used and how recreational ice hockey participation is being impacted by the grant.   They also agree to register all participants in their program, including those impacted by the grant with USA Hockey.

 

APPLICATION INSTRUCTIONS –  Please follow these instructions carefully.  Each application should have five (5) sections as outlined below.  Each section should be on a separate page or pages.   

 

        1. Complete the Cover Page below.

 

  1. Provide a list of current Board members along with their position or area of responsibility.

 

  1. Provide a copy of your current budget, specifically for any and all hockey operations.

 

  1. Provide a brief description or overview of your organization including such things as programs offered, facilities, current efforts to attract new participants, and any other information that you feel would be pertinent in our evaluation of you application.

 

  1. Provide a detailed description of your proposed program or initiative to increase participation in recreational ice hockey.  Include such things as:
    1. Budget for equipment, supplies, services, etc. – include income or funding information.  Will this grant be the sole source of funding for this initiative or are additional funds coming from other sources?
    2. List of personnel anticipated to be responsible for implementation, along with contact information – phone and email.
    3. Facilities involvement.  Is the ice arena a partner in this project?
    4. Target audience – What segment of hockey are you trying to grow?
    5. Plans for marketing or promoting your program.
    6. Timeline for implementation.
    7. Specific goals of the program or initiative – how will success be measured?
    8. Potential pitfalls and plans to address them.
    9. Expected impact on your association.
